STEPS Transitional Recovery and Support Service

Karakan’s STEPS Transitional Recovery and Support Service is a service operating in partnership with Karakan and Metro South Health that aims to work with and empower people to improve their mental wellbeing, quality of life, and long-term housing stability.

We support people who are:

  • 18-65 years old
  • Residing in the Logan catchment
  • Currently receiving support from Metro South Addiction and Mental Health Services
  • At risk of losing their accommodation due to their mental health needs.

Stages of Support

We offer a range of supports through our service, including:

  • Support and Sustain (support at home): This stage involves supporting you in your own home with a Karakan case manager who can work with you for up to 24 weeks, for 5 hours per week, to identify and work towards your recovery goals and help you to achieve long-term, stable housing.
  • Step In (Lyston House): If you would benefit from a higher intensity of case management and support, Lyston House is available to step into as a supported accommodation service. Lyston House is a semi-rural home located in Greenbank, offering lots of space, gardens, a pool and animals as part of the therapeutic experience. Our house can provide up to 12 weeks of live-in support to achieve your recovery and housing goals. It includes regular weekly support from a clinical occupational therapist, Karakan case managers, Karakan recovery workers, and social and group activities to enhance your connections to what matters to you and your community. Lyston House is home to a friendly residential dog and a small number of farm animals.
  • Step Down (low intensity house support): Once you are ready to transition from Lyston House, you will be supported to move into one of the program’s low intensity houses. Our low intensity houses can provide up to 24 weeks of support from recovery workers for 30 hours per week during weekdays to continue to work on your recovery and housing goals. You will also be able to receive support from the clinical occupational therapist and case manager as you continue to build meaningful connections in your life.
  • Step Forward (support at home): This stage involves supporting you in your own home with a Karakan case manager who can work with you for up to 24 weeks, for 3 hours per week, to support your long-term housing and recovery goals.

After experiencing the confronting reality of supporting his brother navigate the mental health system and recognising the critical need for transitional support from hospital to home that was not available, Karakan CEO Cameron Thayer witnessed first-hand the tragic reality that can occur in a person’s life when essential supports are not available. Turning pain into passion, and with a determination to drive change, Cameron spent 18 months learning from the stories of others, identifying the need and demand for transitional recovery accommodation in the Logan region, and advocating for support and investment by community and government.

Karakan has developed an innovative service model that is built on contemporary social service design and recovery-oriented practice principles. This service is based on different evidence-based models of care that are making a difference and is underpinned by the voice and learnings of lived experience. In developing and delivering this service, we continue to learn from and be guided by people who have navigated their own journey of mental illness, recovery and service navigation.

‘Lyston House’ is one of three houses being established to support this new transitional recovery accommodation service. The name symbolises the importance of listening and learning from people with lived experience to guide our approach to mental health support. It is a symbol of the hope that this tailored service can provide to members of our community.
